A few places to check:
1. Check the form. I don’t think emails come to your personal Jetpack email address. When you set the form up you enter which email address the emails should go to.
2. If the above doesn’t work. Check your email plugin. The additional one you’re using for your SMTP settings.September 17, 2015 at 10:23 pm #2319

With all WordPress challenges a standard troubleshooting process would be to reduce the installation back to a vanilla state. If the issue resolves you’d build it back up again until the issue re-appears.September 18, 2015 at 9:33 pm #2328
I removed the square brackets from your email form subject. Not sure, that might have been a problem. Form works for me now. Switched it over to your email address. Give it a try.September 18, 2015 at 9:50 pm #2329
Thread update: We seem to have been successful by removing square brackets that were part of the user inputted subject.
